Registration Information

Due to COVID-19 pandemic and limited access in our building, 

Registration will take place online. You must fill out the registration documents and email them to Ms. Helen at:  HVERVENIOTIS@SCHOOLS.NYC.GOV


Please click on this link:  to DOWNLOAD and SAVE the required documents to your desktop.  These are fillable documents, but you must download and save them to your desktop BEFORE you fill them in.  It's important you follow this step, otherwise you will be emailing blank forms.  


To complete registration you must download and complete the following forms available on the DOE website:

  • Student Registration Form

  • Emergency Contact Form

  • Home Language Identification Survey

  • Parent Ethnicity Survey

  • Housing Questionnaire


 In addition, you will need to scan and email the following required documents:

  • Proof of Age (scan/take picture of birth certificate)

  • Proof of Residency (scan/take picture of TWO proofs of residency)*

  • Immunization Record **

  • Photo Identification of the parent/guardian submitting the paperwork

  • Copy of IEP (if applicable)

  • Report Cards and academic records

SAVE ALL RECORDS AS:  LAST NAME,FIRST NAME FILE NAME  (i.e. Verveniotis,Helen Registration Form)

*PROOF OF RESIDENCY:  visit for a list of acceptable proofs of residency


**IMMUNIZATION RECORDS:  You must submit proof of immunization for your child to attend school. All students must meet the requirements for:  DTaP (diphtheria- tetanus-pertussis), poliovirus, MMR (measles-mumps-rubella), varicella and hepatitis B vaccines.  Visit for more information.
